New patients, please arrive with plenty of time to fill out all registration paperwork before your scheduled appointment or print out the paperwork and fill it out before you arrive.
Finance Information: NTC Community Clinic accepts over 60 kinds of insurance, such as Medicare, Medicaid, and numerous private insurances. We also accept Self-Pay patients and even offer a 10% discount if full payment is made on the day of service. Payment Plans are available under certain conditions.

What to bring to your first appointment:
- Current insurance card(s)
- Medicare card
- Current Medicaid sticker
- Tribal ID card or CIB (if an IHS beneficiary)
- Medication list
Current patients, please arrive no later than 5 minutes to your appointment. If you are unable to make an appointment, please notify us at least 24 hours before your scheduled appointment. If you are more than 15 minutes late to an appointment without giving us advance notice, you will be considered a no-show and your appointment will be cancelled.
Walk-In Patients are welcome; however, unless your condition is deemed an emergency, our scheduled appointments take precedence. Calling and scheduling appointments in advance is the best way to ensure you are seen in a timely manner.
The NTC Community Clinic is pleased to offer our patients 24/7 access to their medical records free of charge. By using our secure Patient Portal, you can have clinical summaries within minutes of your provider finishing documentation of your visit. See your lab results, current and past medication lists, immunization status, and much more. If you have not received access to your medical records via our Patient Portal, please contact the Clinic Receptionist.
